First of all sorry I've not had time to put my 2c in about the rest of
the Drupal stuff. I'm travelling at the mo, and it's going to take some
time to write something thoughtful. This however is pretty quick and easy:-

Any module that calls out, or uses external provision or ip addresses is
basically not an option an Indymedia site. So akismet, mollom, recaptcha
etc. are all out.
The basic captcha system is just that, and has been cracked pretty well.
Although it may still be an option as long as you are giving a variety
of options for users of different abilities.
The http://drupal.org/project/spam module does actually work shockingly
well even without IPs (baysian stats, plus a few crafted rules). The
biggest problem I've found is with a few false positives, which are
harder to follow-up as less people are aware of them.
These along with allowing users to report posts, and allowing more of
them to hide them. You can of course give permissions to do these things
selectively to users, without giving them other admin rights. Increases
participation and reduces the work load for stuff that might get missed
by filters.
