[Oscailt] Banned Author Names extension for Oscailt 3

Implemented banned author names for oscailt 3 : see

Any oscailt coders - would it be possible to get this incorporated into the
main CVS branch?  Sorry this isn't a proper patch, hope this is enough
info.  I can provide the actual files if this helps, or I can patch the CVS
version and submit the patched files if you prefer.

If so I would make a further change to make editbannedauthors a seperate
permission from editbannedips.  Possible extension would be to make the IP
ban time configurable per author name, with 0 being no ip ban.

BTW: neat code base - this was much easier than I expected!
