[Oscailt] problem with notifications

hi,
we have an oscailt 3.0 installation at http://calabria.indymedia.org.
Everything seems to be working fine, except for the fact that
*sometimes* notify e-mails for administrator operations seem not to be
generated, even if the "notify this operation" checkbox is checked.
any suggestion?
