We are still waiting on some suggestion that Amy said she was going to
send.  Here are my edits:

change to make more international:

"3) Are you a non profit corporation / association / do you have a
partnership with a non-profit org to help you accept donations or
manage finances?"

and move to the end of the collective profile section

something ongoing? How many are paid and for what jobs?"

add (Cat's suggestion):
